摘要:
目的 研究晚期糖基化终末产物受体(receptor of advanced glycation endproducts,RAGE)在糖尿病性白内障(diabetic cataract,DC)及年龄相关性白内障(age-related cataract,ARC)患者晶状体前囊膜和不同浓度高糖条件下培养的人晶状体上皮细胞(lens epithelial cells,LECs)中的表达。进一步探讨RAGE在DC患者晶状体前囊膜和人LECs中表达的变化及相互之间的关系。方法 收集DC及ARC患者的晶状体前囊膜,采用Western blot检测RAGE在前囊膜中的表达。ARC患者根据年龄分为:ARC1组(50~69岁)和ARC2组(70~89岁)。DC患者根据年龄及糖化血红蛋白(hemoglobin A1c,HbA1c)水平分为:DC1组(50~69岁,HbA1c为>7.0%~12.0%)、DC2组(50~69岁,HbA1c为4.0%~7.0%)、DC3组(70~89岁,HbA1c为>7.0%~12.0%)、DC4组(70~89岁,HbA1c为4.0%~7.0%)。将培养的人LECs根据培养液不同分为5组,A0组:采用DMEM 培养液;A1组:添加10 mmol·L-1葡萄糖的培养液;A2组:添加20 mmol·L-1葡萄糖的培养液;A3组:添加30 mmol·L-1葡萄糖的培养液;A4组:添加30 mmol·L-1甘露醇的培养液。Western blot检测以上各组中RAGE的表达,流式细胞仪检测以上各组人LECs凋亡率。结果 RAGE在前囊膜的表达程度:DC1组高于DC2组,DC3组高于DC4组,均高于相应年龄的ARC患者。体外实验中,RAGE在A1、A2、A3组中表达高于A0组,差异有统计学意义(P=0.001),也高于A4组,差异有统计学意义(P<0.001);A1、A2、A3组人LECs凋亡率高于A0组,差异有统计学意义(P=0.002),也高于A4组,差异有统计学意义(P=0.002);A1、A2、A3组人LECs凋亡率与培养液葡萄糖浓度呈正相关(r=0.892,P<0.001)。结论 RAGE在DC患者晶状体前囊膜的表达高于ARC患者,同等年龄下HbA1c水平越高RAGE表达越多;葡萄糖浓度越高,人LECs的RAGE表达越多,人LECs凋亡率越高。推测RAGE可能参与了白内障的形成,并加速了糖尿病患者的白内障进展。
Abstract:
Objective To investigate the expression of the receptor of advanced glycation end products (RAGE) in the anterior capsules of patients with diabetic cataract (DC) and age-related cataract (ARC) and in human lens epithelial cells (LECs) cultured under different concentrations of high glucose and the difference and relationship of RAGE expression in DC anterior capsules and LECs.Methods The anterior capsules of DC patients and ARC patients were collected, and the expression of RAGE in the anterior capsules was detected by Western blot. ARC patients were divided according to age: ARC1 group (50-69 years) and ARC2 group (70-89 years). DC patients were divided according to age and hemoglobin A1c (HbA1c) level: DC1 group (50-69 years old, HbA1c was 7.0%-12%), DC2 group (50-69 years old, HbA1c was 4.0%-7.0%), group DC3 (70-89 years old, HbA1c was 7.0%-12%), DC4 group (70-89 years old, HbA1c was 4.0%-7.0%). Human LECs were cultured and grouped into: A0 group: low glucose DMEM culture solution; A1 group: culture solution added with 10 mmol·L-1 glucose; A2 group: culture solution added with 20 mmol·L-1 glucose; A3 group: culture solution added with 30 mmol·L-1 glucose; A4 group: culture solution added with 30 mmol·L-1 mannitol. Western blot was used to detect the expression of RAGE, and flow cytometry was used to detect the apoptosis rate.Results The expression level of RAGE in the anterior capsule in DC1 group was higher than that in DC2 group, DC3 group was higher than DC4 group, and they were higher than the corresponding age ARC group. In vitro experiment, the expression rates of RAGE in the A1 group, A2 group, and A3 group were higher than those in the A0 group, and the difference was statistically significant (P=0.001), which was also higher than that in the group A4, and the difference was statistically significant (P<0.001). The apoptosis rates of the A1 group, A2 group, and A3 group were higher than those of the group A0, and the difference was statistically significant (P=0.002), which was also higher than that of the group A4, and the difference was statistically significant (P=0.002). The apoptosis rates of the A1 group, A2 group, and A3 group were positively correlated with the glucose concentration in the culture medium (r=0.892, P<0.001).Conclusion The expression of RAGE in the anterior capsule of DC patients is higher than that of ARC. At the same age, the higher the HbA1c level is, the more RAGE expression; the higher glucose concentration is, the more RAGE expression in human LECs is, and the higher the apoptosis rate of LECs is. It is speculated that RAGE may be involved in the formation of cataracts and accelerate the progression of cataracts in patients with diabetes.
